Our office is here to serve you. We offer digital x-rays, a calm peaceful environment, friendly service and won't keep you waiting. We currently have hospital privileges at American Fork Hospital, Timpanogos Regional Hospital, Grove Creek Surgery Center and accept most insurance plans.
Our office is conveniently located at 179 North 1200 East, Suite 103 in Lehi, Utah. We are located to the west of Lowe's and just east of Stonehaven Dental.
Appointments
If you would like to make an appointment, please contact our office at (801) 766-9947. If you would like to request an appointment online, please select the ‘Appointment Request’ option from the menu above. A member of our staff will follow up with you directly. We try our best to offer convenient appointment times.
On your appointment day, please bring your insurance card. If your insurance plan requires that you have a referral from your primary care physician, please make sure you obtain one before your appointment. Also, please arrive 10 minutes early.
Financial
Our practice accepts cash, check, and major credit cards. We accept most major insurance providers and will be more than happy to submit your insurance forms for you. We are now also accepting Medicare! Please call our office to verify whether your specific insurance plan is accepted. We also accept cash pay patients that are not using insurance, costs of cash pay visits can be discussed at the time of scheduling as well as at time of service. Payment plans and schedules can be discussed upon request.
Cancellation
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.
